- The list of accepted insurances below is subject to change.
- Please verify that we accept your insurance when you call to make your first appointment, and notify us of any changes in coverage.
- Always bring a copy of your updated insurance and prescription cards to your visits (they are often two separate cards).
- Co-pays are always collected at the front desk before your visit. If it is inadvertently not collected that day, you will receive a bill for the outstanding amount after your visit. Co-pays are determined by your insurance, we have no discretion over when they are required nor the amount collected.
- Patients with a high deductible plan are responsible for all costs until the deductible is met.
- Certain procedures and medications may not be covered by your insurance, we will do our best to notify you if we anticipate something is not covered and partner with you to keep the costs reasonable.
- We accept health savings accounts (HSA), flexible spending accounts, and credit cards.
- We accept self-pay patients, as long as you NOT are covered under a Medicaid-sponsored plan (we are prohibited by the state from seeing Medicaid patients as self-pay). Pricing will be at the discretion of the provider, but costs are similar to what we would be reimbursed by insurance.
